2. Service Level Agreement for CBCT Scans:

Referring practices must complete a Service Level Agreement (SLA) before referring patients for CBCT scans. This agreement only needs to be completed once and once and demonstrates that the referring dentist is qualified to report on the scan.

3. Submission:

Once the forms are completed, they can be submitted via email, or sent to our office in person. Our team will review the referral and contact the patient to schedule an initial consultation.

4. Initial Consultation:

During the consultation, we will conduct a thorough examination and discuss treatment options with the patient. We will also communicate with the referring practice to keep you informed about the treatment plan and any findings.

5. Treatment and Follow-Up:

After denture or dental implant treatment, we will provide a detailed report to the referring practice. We will also schedule any necessary follow-up appointments to ensure the patient’s recovery and satisfaction with the treatment.
